We read nurses notes daily unless covered for rehab but we don’t generally keep them covered just for impaired cognition or behavior unless unstable.

I'm trying to devise a new way of keeping track of who remains eligible for Medicare A.  We have Medicare meeting M,W,F.  At these meeting we discuss who is being billed medicareA, why they are medicareA and if they are still meeting the criteria for medicareA billing.  How does everyone keep track of people who are covered for impaired cognition or for behavior problems?  They have to meet the criteria the entire 100days, not just during the assessment reference period, right?  Are nurses notes being checked daily to make sure they are still meeting the criteria?  I hope I am explained my question well.
