We also do monthly summaries. It's not great for the assessments later in the month 
but we have found that it is better than nothing which is what we were getting most of 
the time. We rely heavily on the assessments that are done by SS and Rec when they do 
their notes for the MDS's to back up the cognitive and several other sections.
