Your pharmacy can provide you with a good one.
We observe three times and then allow
them to self administer. We then have the nurses document weekly how
they are doing and incorporate it into the monthly documentation. The
MDS coordinator evaluates quarterly and annually doing a self administration
assessment for the record.
