Thom,
Do you mean that you want to get the persons email address based around what
ISP they are logged in from?

If so that could never work - as an example you can get a BT broadband
account here in the UK which does not have email facilities at all.
As an example I'm online via BT but I use my infolinkelectronics email for
pretty much everything online, no software you have or could write could
make a connection between the two accounts, either via name or IP so I would
be rejected on your form.
There is no way you can get info like that from the header info that a
browser provides either otherwise spammers would have been exploiting it
mercilessly by now!

You said it was for a feedback script - what are you actually trying to make
it do, in what context?
If it is so important then make people register, that way via a dual hit
authentification you know that the email address they supplied is a valid
one. If it is something sensitive then maybe consider a system where the
feedback post has to be approved by an administrator / moderator before it
is visble on the site.
