If I remember correctly, this happened to me when I ran my app from Xcode but already had the app running. You know, add some code, run the app, correct code, run the app... oops, forgot to quit it first.

Specifically it happens most often when you're running the app in the debugger and then launch it without the debugger. Otherwise Xcode catches it and quits the process (either in the debugger or normally).
