you could broadcast the +v to clients whenever an opped and voiced client 
uses thier opsr loses their ops

eg. if the user is opped and voiced when they are deoped clients will be 
sent

:user2 mode -o+v user1 user1

rather than

:user2 mode -o user1

that should fix the problem
