   The provider changelog generator (`breeze release-management 
prepare-provider-documentation`) decided whether a commit was already listed by 
searching for the literal `(#NNN)`. An entry crediting several PRs at once, 
e.g. `(#73367, #73368)`, therefore looked unlisted and was appended again on 
the next run for the same version. The `check-changelog-has-no-duplicates` prek 
hook had the mirror-image gap: it skipped such entries entirely, so a PR 
repeated across a grouped and a standalone entry was never reported.
   
   Both now extract every PR number from an entry. Grouped references already 
exist in released changelogs, so this is a tooling fix, not a format change.
