   … durable runs

Tool calls that run in parallel took their journal positions in the order
they reached the tool wrapper, which follows how long run hooks, agent hooks
and tool input guardrails took, not the order the model made the calls. When
that timing differed between attempts, the positions no longer matched, the run
diverged and tools that had completed ran again on retry.

The model wrapper now claims one position per function tool call in the
model's own order and each tool picks up its position by tool call id. Also
test the parallel, handoff, streaming and approval cases.
