Added terminal-failure and 404 resubmit cases, a durable=False case that
never touches the store, and a default_args case, all parametrized across both
operators so the `durable=durable` forward is `covered. _state` moved to module
level as `make_run_state` and the `SubmitRun` and `RunNow` copies now use it
too.
Since the predicates are shared after the refactor above, the
lifecycle-state parametrization lives in a new `TestRunStatusHelpers` rather
than a third per-class copy. Verified: removing "BLOCKED" fails 5 tests,
turning the 404 branch into a raise fails 5, and dropping durable=durable fails
2.
