Document Jolokia remote access config for the observability example
The Jolokia agent now binds to localhost by default and its restrictor
rejects non-loopback requests, so hawtio-online on Kubernetes needs
server.host and remote-access-allowed to be set. Added them commented
out to keep the local defaults secure, and updated the README.
