We do have a small regression here. I say small, because any batch that would 
hit this can be easily rewritten in a simpler way. Namely, we hit this if a 
batch contains 2+ statements on the same row and with the same "exist" 
condition, and in every case you can just merge it into a single statement and 
not get this. The example in the description is kind of nonsensical (both 
statements insert the same value, which sounds more like a user error, and if 
the test from CASSANDRA-12649 does this, I suggest having a look at that test), 
but a more meaningful example triggering this could be:

CREATE TABLE t (k int PRIMARY KEY, v1 text, v2 text)
BEGIN BATCH
  INSERT INTO t(k, v1) values (0, 'foo') IF NOT EXISTS;
  INSERT INTO t(k, v2) values (0, 'bar') IF NOT EXISTS;
APPLY BATCH;
{noformat}
but that can obviously be rewritten has simply:
{noformat}
INSERT INTO t(k, v1, v2) values (0, 'foo', 'bar') IF NOT EXISTS;
{noformat}

Still, this is a bug, so attaching fix (with a proper unit test) below:

I'll note that the patch doesn't just remove the assertion in 
{{setConditionsForRow()}} as the assertion is sensible for this method, but it 
makes sure we don't call it when we shouldn't.
