Although I do have an occasional "nodetool repair" running, I've not observed
any correlation between the occurrences of this exception and the running of
the repair.
I've seen this on two boxes in a cluster - all but one instance was on one
node, and this node is by far the busier of the two. This is perhaps not
surprising as it appears to be affected by timing, and the apply() function is
being run in a worker thread.
