All sounds good to me, except:

bq. Maybe a couple JMX hooks to both set quota sizes and to reload quotas from 
CQL (break glass for operators)

Having to broadcast JMX calls across all nodes to enforce a quota change feels 
less than ideal to me, I wonder if we should not make quotas a keyspace schema 
option, so quota updates are automatically broadcasted to all nodes?
