Review Comment:
Renamed to LoadBalancedCassandraClusterInfo and moved it out of the
cloudstorage.coordinated package into bulkwriter, since it isn't specific to
coordinated writes. The concrete type is now selected via
CassandraClusterInfo.create(...).
