The recommended changes look good. One thing that i'm thinking about is should we have a changelog.md file, where we include the same content we included in the pull_request_template.md except we only include a summary of the changes and which issue is fixed. This way we can track all of the changes made in one file, and it provides a quick look up table, where we associate the change with the PR. If we include screenshots and everything, this pull_request_template.md file will become too big too quickly.
