Problem Summary: ColumnMapper coverage was scattered across file reader and
table reader tests, and several mapper-specific behaviors were not asserted
directly. This adds a dedicated format_v2 ColumnMapper unit test file with
categorized coverage for schema projection helpers, nested helper behavior,
root mapping modes, constant and virtual mappings, scan request localization,
complex projection pruning, and split-local schema evolution state. It migrates
existing root mapping, BY_INDEX, and constant filter entry cases into
column_mapper_test.cpp so mapper semantics are covered in one focused file, and
adjusts existing nested projection assertions to match the mapper's current
scan projection contract.
