in geronimo osgi environment, bean manager bean is not getting set up

I'm not sure why this isn't a serious problem in trunk geronimo + openejb, but
in my osgi work I'm not finding that the BeanManager bean is getting set up
during app initialization. The code where it's done in regular OWB lifecycle
doesn't exist in the openejb lifecycle so I'm proposing to put it in the
"setup default beans" code.
