The issue seems to match the problem description of https://github.com/apache/pulsar/issues/21421 . There's a pending fix in Bookkeeper https://github.com/apache/bookkeeper/pull/4196 . This will be included in the 4.16.5 release that is in progress. Bookkeeper 4.16.4 included a fix https://github.com/apache/bookkeeper/pull/4140 in the same area.
