Review Comment:
   Re-confirmed on the current branch: `initializeWithOffset()` now logs an 
error and aborts (no `fillPreBlank()`, no `flush(0)`) when the mapped file 
cannot be created or held, so a file availability problem no longer looks like 
a successful initialization.
