## Why
The create-user modal only closed itself on cancel; it never reset the form
and the form values were preserved. Cancelling after typing a username and an
initial password left both — including the plaintext password — sitting in the
form store, and they reappeared when the dialog was reopened. The sibling
settings modals (DataSourceTab, CloudCredentialTab) already reset on close with
`resetFields` + `preserve={false}` + `destroyOnHidden`.
## What changed
- Cancel now calls `createForm.resetFields()`, the Form uses
`preserve={false}`, and the Modal uses `destroyOnHidden`, matching the
settings-tab pattern.
## Validation
- New test `UserManagementCreateModalResidue.test.tsx`: type
username/password, cancel, reopen — both inputs must be empty. Fails before the
fix (values persist), passes after.
