### What DuplicateInstanceNameException rejects a duplicate instance registration. It had no test. This pins: a duplicate name is a client error (400) and the message names the conflicting instance so the UI can point at it. ### Verification - New tests on unmodified code: **1/1 green**, checkstyle clean. - Discriminating mutation: the status code drifted from 400 (red). - Restored: green.
