ci: tighten the comments on the approvals gate

Same behaviour; the comments were carrying the reasoning of every round
that produced them rather than what a reader needs now. Cut the retelling
and the repetition, kept the "why" that is not inferable from the code —
which revision the config comes from and why the other two are stale, why
a dismissal is not reported as an approval, why an unresolvable merge
group fails closed, and why the veto is a contradiction rather than a
resolution.

The approvals check loses 35 lines, most of them comment.
