BinaryHeap.remove(Object) seems to break heap order

Fixed in nightly builds starting 1/2/04.

Modified iterator remove() to compare repositioned "last" element to its new
parent to decide whether to percolate up or down.  Added percolateUp methods
with starting indexes to support this.
