Different layouts are generally avaialble through the styling of elements, like mentioned in Central requirements > Layout. Left/right pages specific layouting is only required for autogenerated contents like footnotes and page headers, I think. Those information will be available to the content generators, even I am still not sure how to really implement those. For the cover I think the discussed integration of pages from other PDF documents is the most usable way. Maybe we can make this an optional call from the content generators. A default title document can always be generated from the available metadata in the document, with custom styles given by the layout directives mentioned above. The table of contents is another of those auto-generated blocks. I will update the requirements document to clarify on those items.
