situations and you must pass to avoid filling eyes.

Yes, if you want an absolutely complete database, then you can't take
any shortcuts. Beginning with "realistically obtainable" positions
would trim the tree and you can fill the gaps later if needed.

I can't see any use in investigating a game where white passes the
first 20 moves, then black passes 20 times... :-)

The problems in the db can be even more subtile. You can reach
identical boards with different players to play even without pass
moves.

Look at these two games. In particular look at the position after
move 25 in 4628 and move 26 in 17596.

In these games the particulars are irrelevant because the game
result has already been determined, but it points out the problem.
