changed command from sh -c '/usr/..........index.html' to
konqueror /usr/........index.html
Now works in all wm's......
points out that we need a way to "declare" a default browser.... but for now.
I'm claiming it's fixed.

Clicking on any of the above does nothing... No errors, nothing in /var/log
Clicking on QT3 Assistant in the same submenu does work.
Using 9.1 beta 3 with all packages upgraded to Cooker Current.
