Attached are two patches to make option formatting more consistent.

The first ensures more --option=... variations are supported, i.e. italicized.

The second ensures -X[OPTIONAL] only formats the -X as bold,
and puts the "OPTIONAL" in italics, rather than having it all bold.

The first patch addresses the formatting issue reported at:
https://github.com/coreutils/coreutils/issues/84

I remember sending a similar patch [1]. Is this one an improvement over
that? Or did you just forget (no worries, I did too)?

Ugh sorry, I completely forgot about that.

In my mind I expected it to go land upstream in help2man,
which I always sync back before release
(I recently checked there were no changes before the 9.8 release).

Anyway this is a superset patch,
which we can apply locally (and upstream hopefully).

thanks,
Padraig
