Responsible for gathering requirements and creating a functional
requirements document for a new performance management system. The web-based
electronic data collection and reporting system includes various types of
performance data from 34 organizational units. The data system includes 180
unique performance measures that are linked to the agency’s strategic plan.
Staff access the system through the agency’s intranet.  The system was
designed using Microsoft Access and configured so that staff enter data
through a web-based application.  Reports are generated using the Access
database reporting function.

Agency intends to  redesign the PMS to improve its functionality and to
align the performance measures with Healthy People 2010/2020 and the
voluntary national public health accreditation standards and measures.

*This scope of work consists of conducting the joint application design
process to include the following tasks:***

·        Working with the staff to identify project objectives and
limitations

·  Identify critical success factors for the development project and
business function

·  Define the schedule of workshop activities and project deliverables

·  Help identify workshop participants including business users and
Information Systems (IS) professionals

·  Prepare the workshop material consisting of documentation, worksheets and
diagrams

·  Organize workshop activities and exercises

·  Coordinate workshop logistics with staff

·  Prepare a final report for Agency and IS department.

1.      Business process modeling experience

2.      experience working with state and/or federal government

3.      advanced/expert verbal and written documentation skills

*Preferred Skills (rank in order of Importance):*

1.      experience with performance management systems

2.      knowledge of public health agency(s) at the federal or state level
