Sorry for replying to wrong message in the thread - missed the original one.

 From what i remember running courier-imap / maildir / webmail, 2 things 
are usually improve performance if slowing down starts with single-digit 
users:
1. imap-proxy
2. filesystem of NFS server that can do fast listing of directories 
containing thousands of files (we used NetApp filers).

Just my experience though - a lot changed since i'm sure.
