For mutt and other mail readers which handle maildir, you can probably create individual maildirs wherever you feel like. But for the IMAP server you have to create folders within your main Maildir directory. The maildirmake man page describes the distinction between the two (see the places where the -f option is discussed). For what it's worth, I use mulberry on Solaris and Linux, and I haven't had any problems with mailbox visibility, mailbox namespaces, or creating new mailboxes through the IMAP server.
