Try deleting the "maildirsize" file in the maildir, then delivering a new message. If you look in it, you'll find the first line is the quota for the mailbox. Only if the maildir is apparently over-quota. Otherwise it recalculates the quota every ~640 changes, where each change is represented by a line in the maildirsize file. (e.g. courier's deliverquota or maildrop) In other words, maybe qmail is just delivering messages without reference to quota at all.
