You'll need have your authdaemon set up correctly to have this work.  I 
have my authdaemon set up for pam which in turn is using winbind to auth 
against my AD. Works pretty well.

courier/etc/esmtpd:
ESMTPAUTH="LOGIN"
AUTHMODULES="authdaemon"
