That ain't completely right.
Some distributors seem to change the default configuration a bit.

At least in Debian woody (courier 0.37.3) and in the current backports
for woody from backports.org (courier 0.42.2) ESMTP authentication is
disabled by default. Seems like it generally is disabled by default in
any Debian package, for some people seem to see a security risk in
enabling it by default.

[for the list archive, applies at least to Debian GNU/Linux woody:]
For ESMTP authentication to work, one needs to edit /etc/courier/esmtpd
and set the following options to e.g. the following values:
 AUTHMODULES="authdaemon"
 ESMTPAUTH="LOGIN CRAM-MD5"
