I had a similar, not quite the same issue doing the same thing (copying to another server) and it turned out to be duplicate emails, one read and another unread in the same directory. A result of not using the rsync --delete flag on the second pass when the server was offline later that night. This only affected some users, those that had read newly recieved mails between the first and second sync.

Install "fdupes" and run that to check for dupes in the affected users dir, it may help.
