The survey questionnaire on the Ministry website is not carved in
stone. It was designed for a specific purpose, and as Vinod points
out, parts of it are inappropriate in the rural context. However, it
offers a useful template that can be modified for your purpose.

What the applicant should be clear about is the additional information
required under the revised Application Form, which the survey will
address:

16. How your programmes are going to help the targeted community:
17. How you propose to involve the local community in the production
of programmes and management of CRS:
18. Have you made any survey amongst the community about the need for
setting up CRS and the requirements of the targeted population? Give
brief details.

Please keeps these requirements in mind when you design your survey
and conduct it.
