A couple of questions... Does this happen when you have multiple files
already open? How about your Settings of Last File Restore and Tabbed
Documents? When this happens, type Ctrl+L and enter ":buffers!" (on the
command line)... is the file you opened listed in the buffers? If so,
use the menu Window > Tabs > Refresh Tabs. Is your file now listed?

I think there is still a bug in the tab refresh routine, I've
occasionally noticed it with SaveAs.
