This will take you to a drop down box. Choose your institution, complete the rest of the form, and hit submit again. You will get a confirmation page that will be followed by an email with your responses. This is a short survey that is vital to your institution's continued membership in TexShare. By filing your annual report survey with the Texas State Library and Archives Commission, your institution's membership with TexShare will be automatically renewed. (See Texas Administrative Code Title 13, Part 1, Chapter 8, Rule 8.3, section e, 1.) We also use the information from your Annual Report Survey to verify the contact information we have on file for your library. Please complete your Annual Report Survey online by February 1, 2008.
