* pdf/xpdf/pdf-document.cc (pdf_document_get_title): guard against
unloaded docs when the title is accessed.
* shell/ev-password-view.[hc]: New widget for displaying password
state.
* shell/ev-window.[ch]: Refactor password handling code to handle
the new view.
