* providers/mysql/gda-mysql-provider.c: added function
gda_mysql_provider_create_index() and added DROP INDEX/
PRIMARY KEY method to gda_mysql_provider_drop_index().

added parameter 'index_list' and code to create the
indexes.
(get_mysql_tables):
suppress some compiler noise by proper casting of
pointers.
