Log message:
If our fallback attribute db appears to be in a strange state due to
multiple entries for the same directory and filename, don't just freak
out and throw an exception: instead, delete the duplicate records
and just return null.
Don't clear the associated path flag if a read fails: there could be
another path in the db that hashes to the same value.
