* libblam/Makefile.am:
* libblam/blam-channel-menu.[ch]: Removed.
* libblam/blam-spinner.[ch]: Removed.
- Removed C glue code, spinner wasn't used and the channel menu has
been moved to managed code.

* src/egg-sharp.cs:
- Changed private to public on glade callbacks to get rid of a bunch
of warnings.
* src/ChannelList.cs:
- Implement the channel menu in managed code instead.
