* src/menus/menus-file.xml
* src/menus/menus-toolbox-file.xml: removes the xinclude to
file-open-location.xml now, which was accidently not applied by the
last checkin. *lso removed the redundant namespace declaration for
every xinclude.
* stylesheets/plainhtml.xsl.in: the conversion to HTML creates now
HT*L files and uses the id as a filename
