With an outbound only out-of-band header with a BARE method (not wrapped), the
code generator messes up the method signature if -exsh true is specified.
If -exsh false is specified, the code is generated correctly, but the
HolderInInterceptor and HolderOutInterceptor are expecting there to be
parts/holders for those headers and an IndexOutOfBoundsException is thrown.
