I believe that cyber time is used by a number of cafes (in the list
below).
Also, Digital Access has come up frequently here, and from the
feature list, would be at the top of the your list.
http://www.digital-u.com/digitalaccess/index.html it may be a bit spendy
(although they had a sale a while back), but it does a lot.
And then there's the one from the guy who got slammed for promoting it too
heavily here. I think it's in beta now, and since it's 'free', you pay in
some other way - probably banners out the kazoo. I don't have the URL
right now.
