2005 -------
sort of two issues, but closely related imho.
1. edit form in design view. titlebar says "openoffice.org writer".
even though form editor is writer based (it is, right ?), it should have 
different title.

2. if form design view has been used, base closed and writer opened, writer has 
lost all it's toolbars and two toolbars that are needed for form design are 
enabled - "form controls" and "form design". form design view should have 
separate toolbar memory.
