This is because your CSV_XS is segfaulting.  

ActiveState does not have the latest versions of these modules.  Jeff just
released them about a month ago, it usually takes ActiveState half to one
year to update:-)

Just download the source from CPAN and nmake yourself.

If you can't do that for some reason, let me know and I can compile and
release a PPM on my repository.

I removed the old sql-statement and dbd-csv modules and installed the
latest ones with PPM from AS.
