What OS are you running?  I've just setup dbmail+qmail on OpenBSD and I
know that I had to change something when piping the messages to dbmail.

|/usr/local/sbin/dbmail-smtp -d ${RECIPIENT:7}

in your .qmail file.  This needs to be checked to see that it works. 
Looking at the qmail log file I see a message going to
[EMAIL PROTECTED]  When it hits your "inbox" it will check the .qmail
or .qmail-default file and strip the dbmail- off of the address and pipe
it to the dbmail database where you'll have to have [EMAIL PROTECTED] setup. 
You probably know all of this... however, when I setup my system on
OpenBSD I needed to have:

|/usr/local/sbin/dbmail-smtp -d ${RECIPIENT#???????};

Which does the same thing only for a different shell.  Bash works just
fine doing it the first way... sh does not.  Hope this helps.
