I recognize that we only recently added the unique key to enforce this, but why did the client application create a duplicate mailbox in the first place? I think you're going to have to manually repair this; can you move all of the messages from one mailbox to the other? Hopefully few enough people have this problem that we don't have to write lots of code to handle it...
