Some tips:
Increase the logging_level to 511.
Try getting dbmail to work without LDAP first.
Don't expect to be able to use dbmail-users as a full-fledged LDAP
user-management tool. I never use it as such, other than to quickly dump
a list of users and forwards.

It looks like you didn't load sql/mysql/create_tables.mysql correctly.
Or the database user doesn't have proper permissions.

The ldap configuration looks ok. But it's impossible to tell if they are
valid for your ldap installation. Is the dbmail.schema loaded by your
ldap server? Does the cn=admin user really have full write access to the
ou=users subtree?

I'm sorry, but that is not a LDAP configuration. It's just the base
object of your users' subtree.
