if the local error indicators are rotationally symmetric, then also the refined grid should be rotationally symmetric. So there must something go wrong in your code. Perhabs you could provide us with some more information: Where does it go wrong? Check the error indicators and do refinement by hand. Are there differences between your refined grid and the automatically generated one?

The square grid geometry is not a problem. If the boundary conditions are fine, then this also works for rotationally symmetric solutions.
